  3. Bank Clearance (CMFB)
  4. Alumni Association Check (Payment of N4000 into CU Alumni Acct 9406011 at CMFB for Alumni Dues for One year)

  In the event of a loss of either (1) or (2) above, please secure a police report and a sworn affidavit to that effect.
